There is a bug that is allowing, on rare occasions, the same destination domain to show in both the "allowed" and "blocked" sections of the menu.

I've noticed this a few times and it has also been reported by two users here:

I still have not found a repeatable case of it.

Going forward, I think what I'll need to do is add in some code to check for this particular situation and log the relevant info when it happens.

Seem to encounter this frequently and at different levels of specificity.
